WHAT TO BRING FOR YOUR CONSULTATION
To allow us to know your health status and your current medical complaint better, it would be helpful if you can bring all available past medical records for your first visit with us.
Relevant and useful information include the following:
Please bring along all the medication, or the names of the medication that you are currently taking. Important information for each medication includes:
- The name of the medication
- The dosage of the medication
- The frequency e.g. two times per day, three times per day, etc
- We would also need to know if you are allergic to any drugs. If you have a history of drug allergy, please tell us the name of the drug, as well as the type of allergic reaction you had experienced e.g. rash, asthmatic attack, etc.
Please bring along all your previous laboratory results, including the results of blood tests and urine tests. In addition, if you have had previous surgical operations or procedures such as endoscopy performed, do bring along any operation or endoscopy reports that you may have. Also bring along all pathology reports.
Please bring along the reports of all radiological investigations that have been performed. Examples of radiological investigations include X-ray studies, Ultrasound scans, CT scans, MRI scans and PET scans. If you have the films or the CD-ROM on which the images are stored, please bring them along.
